Welcome to Invent Health (#inventhealth), a twilight salon-styled tech talk and mixer produced by Vator and UCSF Health Hub, where valuable lessons and insights are shared by investors, tech pioneers and adopters. Invent Health is designed to take a 360-degree view of one particular topic. 

In this case, the topic is “Primary Care and the New Medical QB.” Primary care physicians are reporting burnout at twice the rate of other healthcare caregivers in primary care practices. The main reason is because they’re seeing 20 patients a day while now having the added burden of inputting their interactions into electronic health records. Adding insult to injury, they’re one of the lowest-paid doctors. This is reflected in the little amount spent for primary care. A 2019 study found that under 6% of healthcare spending went to primary care in the US compared to 14% for other OECD countries on average. The same report by The Robert Graham Center, which looks at policies around primary care, showed that increased investment in primary care reduces total healthcare spending. For example, in Rhode Island, a four-year effort to cap prices and expand primary care resulted in an increased primary care spend of $27 million but an overall reduction in costs of $115 million. 

And yet, primary care physicians are the gateway to our health. Consider that 79% of antidepressants are prescribed by doctors, who more often than not prefer to spend more time with patients than just administer medications. As Dr. Archana Dubey says, the primary care system is “run like an assembly line and is an upside-down pyramid scheme of a model!”

So how do we change it? Many companies are trying to figure that out. K Health wants to provide primary care at $9 a month for 24/7 unlimited access to a clinician. In-person clinics, like One Medical and Forward, are reinventing the look and feel of a clinic so it’s more technologically-driven but also as inviting as a spa. The Well is another take on primary care focused largely on prevention. There’s also the telehealth-first approach with companies such as Doctor on Demand, Amwell and Teladoc, which is aggressively building upon its telehealth reach with its acquisition of Livongo, and its explosive BetterHelp division. Then you’ve got other emerging companies such as Ro and Hims/Hers, which like K Health, focuses on cash pay and specific conditions, like sexual health and smoking cessation.

It’s an evolving world with the primary care doctor potentially at the center as the new-age quarterback.   

In 10 years, what will the doctor's office look like? Will it be integrated with your gym? What's the role of the medical doctor? How much will they matter if people self-diagnose already, and are now seeking more guidance about nutrition, mental health, lifestyle changes?
